Identify your team

Tean-based care is a cornerstone of the PCMH model.  Whether your practice is a staff of 3 or 20, you can become a well-functioning team by adopting some or all of these key competencies:  

  • Focus on patient-centered care
  • Culture where staff feels respected and valued
  • Common goals for the patient and practice
  • Shared responsibility for patient outcomes
  • Understanding of professional roles and responsibilities of each team member
  • Ability to trust the work of others
  • Team competencies that reinforce each other
  • Ability to be flexible and adapt to bumps on the path to PCMH
