Advance your understanding of new models in chronic disease management

  • A Chronic Care Model(CCM) has been developed and tested as a means of improving the delivery of excellent care. It has now been shown to work effectively in hundreds of diverse settings. Elements of this model are reinforced throughout this resource center:
    • Patients with chronic illness are encouraged to participate in setting goals and monitoring their treatment
    • Clinical staff members have defined roles and tasks and follow up with patients regularly to support them in managing their diseases
    • The health care team knows about existing community resources that can help patients and encourage their participation
    • Clinicians use explicit guidelines for patients’ medical and lifestyle issues
    • The office uses computers efficiently to support clinical decisions and streamline practice
    • Performance improvement and the pursuit of excellence are ongoing efforts
