Answer: The patient!

In building your health care team, be sure to incorporate strategies that empower patients as partners. Patients need to “own” their health and become active participants in decisions about their care. They will need your professional guidance to manage potentially complicated medication regimens, learn techniques to monitor their health, and identify strategies to make necessary lifestyle changes.
